ALRC <br /> Active living Ramsey County <br /> Instructions <br /> When completing the application refer to the documents"Criteria for Bench Corridor Projects," <br /> Bench Site Recommendations," and <br /> A. List the name of the local government sponsor. It may be a City or Township, School District <br /> or Watershed District. <br /> B. List the Community Partners, attaching a brief description of their role, addressing the <br /> following: <br /> a_ Type of organization (e.g. non-profit; public corporation, privately held firm, fraternal <br /> organization, etc.) <br /> b. Contact name, address, telephone number and email address for the organization <br /> c. Web site, if available <br /> C. Provide the name of the person in the local government authorized to speak on behalf of the <br /> project. <br /> D. Generally define the geographic boundaries of the project, including nearby streets. <br /> E_ Attach the requested documentation. Please note that the primary role of Ramsey County is <br /> to fund the project, and to facilitate bench, paver and concrete pad procurement. The local <br /> government is responsible for addressing pathway planning, bench location, property issues, <br /> permitting, contractor coordination, installation and long-term maintenance of the benches. <br /> F. Identify the expenses related to the entire project. The total cost for each bench, including <br /> installation and message pavers is $ . These are based on contract prices as follows: <br /> a. Benches: $ each, including delivery. <br /> b. Pavers: $ each; three pavers per bench installation <br /> G. Concrete slab: $ if using the vendor under contract to Ramsey County <br /> An applicant is obliged to use the benches and pavers procured by the County, but may use <br /> a different entity to install the concrete pad and install pavers and benches. The concrete pad <br /> must be installed according to specifications provided by the County. <br /> G. Active Living Ramsey County will provide 50% of the cost up to $600.00 per installed bench <br /> site. Identify specific sources and amounts of other public and/or private funds. <br /> H. Supply documentation of public right of way license or permit for each bench site within the <br /> proposed network. <br /> I.The local government sponsor is responsible for the ongoing maintenance of benches. <br /> Identify who will provide these services and the long-term source of funding for maintenance. <br /> Submit the completed application and supporting documentation to: <br /> Mary Elizabeth Berglund <br /> Saint Paul—Ramsey County Department of Public Health <br /> 2285 White Bear Avenue <br /> Suite 350 <br /> aplewood, MN 55109 <br />
