City of Arden Hills Personnel Policy Effective August 26, 2003 <br /> <br />FAMILY AND MEDICAL LEAVE <br /> <br />In accordance with the federal Family and Medical Leave Act (FML <br />leave will be granted to all eligible employees (male and female for up to twelve (12) weeks <br />per twelve (12) month period for any of the following reasons: <br /> <br />1. Birth, adoption or foster care placement of a child; <br /> <br />2. To care for a spouse, child or parent who has a serious health c <br /> <br /> or <br /> <br />3. For a serious health condition that makes the employee unable to the <br />functions of the position. <br /> <br />Eligibility <br /> <br />An eligible employee is one who has worked for the City for at least: <br /> <br />1. 12 months, and <br /> <br /> <br />2.1,250 hours during the twelve month period prior to requesting t <br /> <br />Length of Leave <br /> <br />The length of FMLA leave is not to exceed twelve (12) weeks in a(12) month <br />period. The entitlement to FMLA leave for the birth or placement of a ch <br />(12) months after the birth or placement of the child. <br /> <br />Leave Year <br /> <br />The leave year will commence the first day the employee is absent from work on FMLA. <br /> <br />Substitution of Paid Leave Time <br /> <br />An employee may request or the employer may require that an empl <br />Personal Time Off for all or part of a family/medical leave taken for any reason. <br /> <br />Notice <br /> <br />1. The employee is to give notice at least thirty (30) days prior to the date on <br />which leave is to begin or if thirty (30) days notice cannot be as much <br />notice as practicable. <br /> <br /> 30 <br />
