Page 1 of 2 <br /> <br />DATE: December 8, 2014 <br /> <br />TO: Honorable Mayor and City Councilmembers <br />Patrick Klaers, City Administrator <br /> <br />FROM: John Anderson, Assistant City Engineer <br /> <br />SUBJECT: Round Lake Road Area Improvement - Payment #6/Change Order #1 <br /> <br />Requested Action <br /> <br />Approve Change Orders #1 andPayment #6 to Meyer Contracting, Inc., in the amount of <br />$155,479.59 for the Round Lake Road Area Improvement. <br /> <br />Background <br /> <br />The City Council awarded the Round Lake Road Area Improvement to Meyer Contracting, Inc., <br />on May 12, 2014, in the amount of $1,766,605.41. <br /> <br />Discussion <br /> <br />This sixth pay request includes work through November 28, 2014. Five percent is being <br />withheld from the work completed according to the contract documents. The payment is in the <br />amount of $155,479.59. Major items of work for this payment are: <br /> <br /> storm sewer <br /> gravel base <br /> subgrade preparation <br /> bituminous paving <br /> <br />Change Order #1 is for additional work fusing nine joints in the 12” water main on the project.
