Submit the following information per Rule DA: <br />c. Name, address, and phone number of party responsible for maintenance of all erosion <br />and sediment control measures. <br />e. Revise the erosion control plan sheet to include the proposed location of the rock <br />construction entrance. <br />h. Provide documentation that an NPDES Permit has been applied for and submitted to the <br />Minnesota Pollution Control Agency (MPGA) <br />i. Submit a copy of the Stormwater Pollution Prevention Plan for the project. <br />Administrative <br />2. Send one final, signed full sized plan set to the District, and email a pdf copy to both the District <br />and the District Engineer. <br />3. The applicant must submit a draft legal document providing satisfactory form and description <br />granting ponding and flowage easements over all onsite ponds, wetlands, and other hydrologic <br />features if the municipality with jurisdiction requires such an easement. <br />4. The applicant must submit a Draft Declaration for Maintenance of Stormwater Management <br />Facilities acceptable to the District for the proposed onsite stormwater management and <br />pretreatment features. <br />5.
