placing equipment, vehicles, or other obstructions described therein on the <br /> Right-of-Way for the duration specified therein. <br /> No person may excavate or obstruct the Right-of-Way beyond the date or <br /> dates specified in the permit unless such person (1) makes a supplementary <br /> application for another Right-of-Way permit before the expiration of the <br /> initial permit, and(2) a new permit or permit extension is granted. <br /> Permits issued under this Chapter shall be conspicuously displayed at all times <br /> at the indicated work site and shall be available for inspection by the City <br /> inspectors and authorized City personnel. <br /> 360.3.08 Permit Applications. Application for a permit is made to the City. Right-of-Way <br /> permit applications shall contain, and will be considered complete only upon <br /> compliance with, the requirements of the following provisions: <br /> A. Registration with the City pursuant to this Chapter. <br /> B. Submission of a completed permit application form, including all required <br /> attachments, and scaled drawings showing the location of all existing <br /> roadways and public or private utilities, area of the proposed project, and the <br /> location of all existing and proposed equipment. <br /> C. Payment of all money due to the City for: <br /> 1. Permit fees and costs due; <br /> 2. Prior obstructions or excavations; <br /> 3. Any loss, damage, or expense suffered by the City as a result of <br /> applicant's prior excavations or obstructions of the Rights-of-Way or <br /> any emergency actions taken by the City; and <br /> 4. Franchise fees, if applicable. <br /> D. When an excavation permit is requested for purposes of installing additional <br /> equipment, and the existing Restoration Bond is insufficient with respect to <br /> the additional equipment, the posting of an additional or larger Removal Bond <br /> for the additional equipment may be required. <br /> 360.3.09 Issuance of Permit; Conditions <br /> A. If the City determines that the applicant has satisfied the requirements of this <br /> Chapter, the City may issue a permit. <br /> B. The City may impose any reasonable conditions upon the issuance of a Permit <br /> and the performance of the applicant thereunder in order to protect the public <br /> health, safety and welfare, to ensure the structural integrity of the Right-of- <br /> Way, to protect the property and safety of other users of the Right-of-Way, to <br /> 360-9 <br />
