Page 1 of 2 <br /> <br />DATE: November 30, 2015 <br /> <br />TO: Honorable Mayor and City Councilmembers <br />Sue Iverson, Acting City Administrator <br /> <br />FROM: John Anderson, Acting Public Works Director <br /> <br />SUBJECT: 2015 Pavement Management Program (PMP) <br />Pay Estimate #6 and change order No. 2 <br /> <br /> <br />Requested Action <br /> <br />Approve Payment #6 to Valley Paving, Inc. in the amount of $913,366.11 for the 2015 PMP <br />Improvements. <br /> <br />Background <br /> <br />The City Council awarded the 2015 PMP Improvements to Valley Paving, Inc. on May 4, 2015, <br />in the amount of $3,826,471.88. <br /> <br />Discussion <br /> <br />This sixth pay request includes work through November 6, 2015. Five percent is being withheld <br />from the work completed according to the contract documents. The payment is in the amount of <br />$913,366.11. Major items of work for this payment are: <br /> <br /> Lake Lane construction <br /> Final wear paving (Grant / Noble) <br /> Final wear paving (Venus/Crystal) <br /> <br />Change Order No. 2 changes the much material from Type 5 to Type 8. The mulch used in the <br />hydro seeding is being changed to a heavier mulch, better suited to the soils, and should produce <br />better growth and turf establishment.
