Time and Talent -A Program for Medtronic Employee and Retiree Volunteers <br />http://intranet.medtronic.conVfoundation/time.htmi <br />year to the budgeted amount. You may apply for a grant if you have <br />been a regular full-time or part-time employee for six months. Retirees <br />and members of the Board of Directors are also eligible for grants. <br />Employees must be classified as an employee at the time of payment of <br />the Foundation match to participate. If you have donated at least 40 <br />hours of time within the past year to a nonprofit organization, you may <br />apply for a grant for that organization once during the fiscal year. <br />Which Organizations are Eligible? <br />ONLY organizations classified as nonprofit 501(c)(3) charitable <br />organizations under IRS tax codes, or governmental agencies, are <br />eligible. Organizations not eligible for grants under the Time-n-Talent <br />Fund include political or religious groups, fraternal organizations, and <br />some civic clubs and professional organizations. <br />What is the Application Procedure`' <br />Complete Part A and forward the entire form to the nonprofit <br />organization for verification. (If you wish to make a financial <br />contribution, include the gift with the form.) The nonprofit sends the <br />completed form, along with the required verification of the nonprofit <br />status, to the Medtronic Foundation. <br />You may personally present the check to your nonprofit organization or <br />the Medtronic Foundation will mail it directly to the organization. Grant <br />payments are made three times each year, in March, July and <br />November. In order for the contribution to be matched in the current <br />fiscal year, the Foundation must receive the completed form by March <br />15.
