Time and Thlent-A Program for Medtronic Employee and Retiree Volunteers <br />F T-- - P, 9`r %,& . kt <br />httpJ/intranet.medtronic.com/foundation/time.htnil I <br />11 <br />• <br />• <br />A Program for Medtronic Employee and Retiree <br />Volunteers <br />What is the Time-n-Talent Fund? <br />Who is Eligible? <br />The Medtronic Foundation is committed to the communities where <br />Medtronic employees live and work. Volunteer efforts are vital to the <br />health of those communities. The Medtronic Foundation's <br />Time-n-Talent Fund reinforces the volunteer involvement of Medtronic <br />employees, retirees and members of the Board of Directors by providing <br />financial support to nonprofit organizations where they volunteer. <br />How Does the Fund Work? <br />The Medtronic Foundation will donate $500 to a qualifying nonprofit <br />organization to which you have given at least 40 hours of volunteer <br />service within the past 12 months. Employees, retirees and members of <br />the Board of Directors who are involved in more than one organization <br />may receive more than one grant per year as long as the volunteer meets <br />the 40-hour time commitment for each organization. <br />If the qualifying volunteer satisfies the 40-hour time commitment and <br />also makes a financial contribution to the same organization, the <br />Medtronic Foundation will match the financial contribution. The <br />minimum contribution to an organization that will be matched is $25 <br />and the maximum is $500. <br />For Example: <br />Foundation <br />Volunteer contributes 40 hours $500 <br />Volunteer contributes $250 $250 <br />TOTAL FOUNDATION MATCH $750 <br />While the Medtronic Foundation does not expect Time-n-Talent Fund <br />requests to exceed the amount budgeted for the program, the Medtronic <br />Foundation reserves the right to restrict the total gifts matched in any <br />A-SA-P <br />• <br />1 of 3 3/13/2000 4:58 PM <br />
