Joynes, Sr., City Administrator <br />FROM: Sue Polka, Public Works Director/City Engineer <br />SUBJECT: Shorewood Drive Mill & Overlay Final Payment #2 <br />Requested Action <br />Approve Final Payment #2 to Midwest Asphalt Corporation in the amount of $4,068.71 <br />for the Shorewood Drive Mill & Overlay Improvements. <br />Background <br />The City Council awarded the Shorewood Drive Mill & Overlay Improvements to <br />Midwest Asphalt Corporation on September 26, 2017, in the amount of $79,694.30. <br />Discussion <br />This pay estimate reflects additional work that was completed, Change Orders 1 and 2 <br />and the release of the 5% retainage. Change Order 1 in the amount of $1,680.00 was for <br />the installation of 450 LF of bioroll that was needed for erosion control due to a rainfall <br />event that took place prior to curb installation. Change Order 2 in the amount of $135.00 <br />was for adjusting a manhole to match the street grade. Final payment #2 is in the amount <br />$4,068.71. The Contractor has provided all necessary closeout documentation. Staff <br />recommends approval of the final payment. <br />Financial Implications <br />With the final payment, the total amount paid to the contractor is $81,509.30 which is <br />$1,815.00 over the awarded contract amount.
