Page 1 of 2 <br /> <br />CONSENT ITEM – 7E <br /> <br />MEMORANDUM <br /> <br />DATE: June 24, 2019 <br /> <br />TO: Honorable Mayor and City Councilmembers <br /> Dave Perrault, City Administrator <br /> <br />FROM: Todd Blomstrom, Public Works Director/City Engineer <br /> <br />SUBJECT: 2018 Street and Utility Improvement Project – Change Order No. 6 <br /> <br />Budgeted Amount: Actual Amount: Funding Source: <br /> $3,300,000 $3,125,099.23 PIR, Special Assessments, Utility Funds <br /> <br />Council Should Consider <br />The Council is requested to approve Change Order No. 6 for the 2018 Street and Utility <br />Improvement Project for retaining wall installation along Indian Oaks Trail in the amount of <br />$39,324.30 (Attachment A). <br /> <br />Background/Discussion <br />On March 26, 2018, the City Council adopted Resolution 2018-034 Awarding the 2018 Street <br />and Utility Improvement Project to Northwest Asphalt, Inc. in the amount of $2,491,595.86. To <br />date, five change orders have been approved totaling $82,048.53 bringing the revised contract <br />total to $2,573,644.39. Eight pay vouchers have been approved with $2,192,760.42 paid to date. <br /> <br />With the installation of curb and gutter and revised grades on Indian Oaks Trail, grading behind <br />the curb at 4235 Hamline Avenue would require the removal of several mature trees and <br />landscaping. In working with the property owner, it has been determined that a short retaining <br />wall would be an acceptable solution to preserve the landscaping. The retaining wall is proposed <br />to be placed 2 feet behind the curb. Concrete will be installed between the back of curb and face <br />of retaining wall. Northwest Asphalt has provided a quote in the amount of $39,324.30. WSB <br />has reviewed the change order and has provided a letter recommending approval (Attachment B). <br />If the change order is not approved, there will be costs to remove trees, landscaping and provide <br />restoration to the area. <br /> <br />
