CONSENT AGENDA — 6D <br />,-i DEN HILLS <br />MEMORANDUM <br />DATE: October 14, 2019 <br />TO: Honorable Mayor and City Councilmembers <br />David Perrault, City Administrator <br />FROM: David Swearingen, Senior Engineering Technician <br />Todd Blomstrom, Interim Public Works Director/City Engineer <br />SUBJECT: Colleen Avenue Storm Drainage Improvements, Payment No. 1 and Change <br />Order No. 2 <br />Budgeted Amount: Actual Amount: Funding Source: <br />$325,389.00 $330,645.00 Surface Water Fund <br />Council Should Consider <br />The council is requested to approve the following items for the Colleen Avenue Storm Drain <br />Improvements project: <br />Payment No. 1 in the amount of $129,066.56 <br />Change Order No. 2 in the amount of $2,456.00 <br />Background/Discussion <br />On June 10, 2019 the City Council adopted Resolution #2019-020 awarding the Colleen Avenue <br />Drainage Improvements contract to Pember Companies, Inc. in the amount of $250,247.00. The <br />contractor began removals of the project on September 13, 2019 and since has completed 50% of <br />the work. The directional drilling operation and storm sewer installation along Colleen Avenue is <br />complete. Placement of granular and aggregate base, curb and gutter installation, a storm culvert <br />and reconstruction of the trail section is underway. Project progress Payment No. 1 is provided <br />in Attachment A. <br />During removal of the existing storm sewer in Colleen Avenue, existing draintile was discovered <br />to run the length of the project limits in the granular section of the road along the curb and gutter. <br />Due to the location of the draintile in the trenched area for storm sewer construction, the draintile <br />was not salvageable and had to be replaced. Draintile was also replaced near the additional catch <br />basin on the south side of Colleen Avenue with the remainder salvaged. Change Order No. 2 <br />includes $2,456 for installation of 300 feet draintile at a cost of $7.52 per linear foot and $100 <br />per connection to structure as provided in Attachment B. <br />Page 1 of 2 <br />
