CONSENT ITEM — 7K <br />, AZEN HILLS <br />MEMORANDUM <br />DATE: September 23, 2019 <br />TO: Honorable Mayor and City Councilmembers <br />David Perrault, City Administrator <br />FROM: Todd Blomstrom, Interim Public Works Director/City Engineer <br />SUBJECT: 2018 Street and Utility Improvement Project — Change Order No. 7 and <br />Payment No. 9 <br />Budgeted Amount: Actual Amount: Funding Sources: <br />$3,300,000 $2,615,187.61 PIR, Special Assessments, <br />(Contractor only) Utility Funds <br />Council Should Consider <br />The City Council is requested to approve: <br />• Change Order No. 7 for the 2018 Street and Utility Improvement Project in the amount of <br />$2,218.92. <br />• Payment No. 9 for the 2018 Street and Utility Improvement Project to Northwest <br />Asphalt, Inc.in the amount of $109,536.34. <br />Background/Discussion <br />On March 26, 2018, the City Council adopted Resolution 2018-034 Awarding the 2018 Street <br />and Utility Improvement Project to Northwest Asphalt, Inc. in the amount of $2,491,595.86. The <br />recommended Change Order No. 7 is for replacing one -gallon perennial planting items with a <br />five -gallon shrub item, resulting in a net cost increase of $2,218.92. Payment No. 9 includes <br />pavement patching, concrete work, turf restoration (sod), and reconciliation of payment <br />quantities. <br />Final wear course paving is occurring the week of September 161 and the project is approaching <br />substantial completion. <br />Page 1 of 2 <br />
