City of Arden Hills Personnel Policy'_` Effective February 29, 2016 <br />ri` 1 ILN HILLS Amended December- ' n October 28 2019 <br />1. All accidents involving city vehicles should be reported to your immediate <br />supervisor and the Difeetev of ° d.. if-ii .tr. , titre Se fv ee- Cid, Administrator, or <br />their designee,'- office as soon as possible. <br />2. Insist that all parties and property concerned remain at the scene of the <br />accident. <br />3. Obtain identifying data from the driver of the other vehicle (name, address, <br />and insurance company). <br />4. Secure names and addresses of injured persons and any witnesses to the <br />accident. <br />5. All accidents and/or injuries must be reported on a form supplied by the City <br />and should be returned as soon as possible from the time of the accident. <br />6. Take a picture of all vehicles involved with cell phone if possible. <br />Safety Equipment/Gear <br />Where safety equipment is required by federal, state, or local rules and regulations, it is a <br />condition of employment that such equipment be worn by the employee. <br />Unsafe Behavior <br />Supervisors are authorized to send an employee home immediately when the employee's <br />behavior violates the City's Personnel Policies, department policies, or creates a potential <br />health or safety issue for the employee or others. <br />WORKERS COMPENSATION REPORTING <br />Workers Compensation benefits and OSHA (job safety laws) require that all on the job <br />accidents, sickness, or loss of consciousness be reported as soon as possible by the <br />employee, or on behalf of a sick or injured employee, to his or her Supervisor. <br />EMPLOYEES MUST REPORT TO THEIR SUPERVISOR ALL JOB ACCIDENTS, <br />SICKNESS, OR LOSS OF CO CIOUSNESS AS %XDN AS POSSIBLE. <br />74 <br />
