Page 1 of 2 <br /> <br />DATE: January 13, 2020 <br /> <br />TO: Honorable Mayor and City Councilmembers <br />David Perrault, City Administrator <br /> <br />FROM: David Swearingen, Senior Engineering Technician <br />Todd Blomstrom, Public Works Director/City Engineer <br /> <br />SUBJECT: Trunk Highway No. 10 Change Order No. 5 <br /> <br />Budgeted Amount: Actual Amount: Funding Sources: <br /> $500,000.00 $622,369.20 State of Minnesota Grant Funding, <br /> Water Utility Fund <br /> <br />Council Should Consider <br /> <br />The City Council is requested to approve Change Order No. 5 for the Trunk Highway 10 <br />Watermain Improvements Project in the amount of $2,645.61. <br /> <br />Background <br /> <br />On October 22, 2018, the City Council approved Resolution 2018-060 approving the <br />Cooperative Agreement No. 1031738 with the State of Minnesota, Department of Transportation <br />for the City’s share of the frontage road and watermain construction along Trunk Highway No. <br />10 from CSAH 96 to Trunk Highway No. 35W. The estimated City contribution at the time of <br />approval was $413,181.89. <br /> <br />The project began April 15, 2019 and since has been substantially completed. Meyer Contracting <br />has completed all City punch list items and only final restoration of vegetation remains. <br /> <br />After the frontage road was paved, core samples were taken of the bituminous surface to test for <br />adequate density. The construction contract includes a provision that provides an incentive <br />payment to the contractor for exceeding minimum compaction density for bituminous pavement. <br />MnDOT has evaluated the pavement material quality and bituminous density in accordance with <br />specifications and special provisions and has determined the results exceeds expectations and <br />authorized payment of incentives at a cost of $2,645.61 provided in Attachment A.
