Page 1 of 2 <br /> <br />CONSENT ITEM – 6J <br /> <br />MEMORANDUM <br /> <br />DATE: September 13, 2021 <br /> <br />TO: Honorable Mayor and City Councilmembers <br /> Dave Perrault, City Administrator <br /> <br />FROM: David Swearingen, Interim Public Works Director <br /> <br />SUBJECT: 2021 PMP Street and Utility Improvements – Payment No. 3 & Change Order <br />No. 2 <br /> <br /> Budgeted Amount: Actual Amount: Funding Source: <br /> $3,807,000 $3,423,767.10 PIR, Special Assessments <br /> Utility Funds <br /> <br />Council Should Consider <br /> <br />Motions to approve, table, or deny the following: <br /> <br />• Change Order No. 2 – Remove and Replace sanitary sewer structure top cone section <br />in the amount of $2,450.00 <br />• Payment No. 3 for the 2021 PMP Street and Utility Improvements Project to C&L <br />Excavating in the amount of $742,080.02 <br /> <br />All items need a simple majority for action unless otherwise noted. <br /> <br />Background/Discussion <br /> <br />On April 12, 2021, City Council approved Resolution 2021-021 Awarding the 2021 PMP Street <br />and Utility Improvements construction contract to C&L Excavating, Inc. <br /> <br />During construction of the street section of Glenpaul Avenue and Prior Avenue, it was <br />determined by the field Engineer that the concrete cone section of the sanitary sewer structure <br />was in poor condition and should be replaced prior to paving the road. There is not a bid item for <br />this work, so C&L provided a price to remove the existing cone section and install the new cone <br />section. Change Order No. 2 for this work is $2,450.00. Bolton & Menk and City Staff <br />recommend approving Change Order No. 2. <br /> <br />The project is approximately 75% complete with the utility improvements completed across the <br />entirety of the project.
