Minnesota Public Facilities Authority <br />GENERAL INFORMATION <br />Recipients of a special appropriation grant from the 2023 Minnesota Legislature are required to enter into <br />a grant agreement with the Minnesota Public Facilities Authority (PFA) for the project funding. PFA needs <br />information and documentation about the project before a grant agreement can be prepared. This packet <br />provides information on the process and the project requirements for State general obligation bond <br />funded projects. It also includes forms and a checklist to help you prepare and submit the information. <br />This packet includes: <br />• Special appropriation forms (fillable PDF) <br />• Sources/Uses form (excel) <br />• Checklist of documents to submit <br />• Example application resolution <br />• State prevailing wage contract language <br />• Example declaration form and waiver request <br />Return completed forms, supporting documents and resolution to your loan officer via email.
