B. Monthly Incident Report. On or before the 151h day of each month, a monthly Incident Report which includes Fire, EMS and Rescue calls. This report shall include at least the following information: 1. Incident Date; 2. Incident Number; 3. Incident Type Code; 4. Alarm Time, Arrival Time and Clear Time; 5. Primary Station; 6. Street Number, Street Name; 7. City; 8. Property Loss; 9. Content Loss; 7.0 Mutual Aid Agreements. Fire Department shall maintain its membership in the Capital City Mutual Aid Association and shall enter into such mutual aid agreements as it deems appropriate and necessary. Such agreements shall be premised on the condition that equipment or personnel will not be sent outside the Fire Service Area if the Fire Department's Fire Chief, or the Chiefs designee, determines that it is needed in the Fire Service Area. Copies of all mutual aid agreements shall be filed with City and Fire Department shall keep City informed of the requirements, rules and regulations of all mutual aid agreements to which it is a party. 8.0 Annual Submittals. The following items shall be submitted to City; A. Operating Budget. On or before July 1s' of each year, Fire Department will submit an annual operating budget for the period January 1 through December 31 of the following year. The operating budget will be itemized for each station. City will make Bi-Annual operating budget payments on or before January 151h and July 151h B. Fire -Year Capital Improvement Budget. On or before July 15S of each year and along with the operating budget, Fire Department shall submit a five-year capital improvement budget which itemizes vehicles and equipment which are estimated to cost more than $10,000.00. C. Annual Audit. On or before July 15S of each year, Fire Department shall provide a copy of the previous year's annual certified audit. 4
