B. Monthly Incident Report. On or before the 151h day of each month, a monthly <br />Incident Report which includes Fire, EMS and Rescue calls. This report shall include <br />at least the following information: <br />1. Incident Date; <br />2. Incident Number; <br />3. Incident Type Code; <br />4. Alarm Time, Arrival Time and Clear Time; <br />5. Primary Station; <br />6. Street Number, Street Name; <br />7. City; <br />8. Property Loss; <br />9. Content Loss; <br />7.0 Mutual Aid Agreements. Fire Department shall maintain its membership in the Capital <br />City Mutual Aid Association and shall enter into such mutual aid agreements as it deems <br />appropriate and necessary. Such agreements shall be premised on the condition that <br />equipment or personnel will not be sent outside the Fire Service Area if the Fire <br />Department's Fire Chief, or the Chiefs designee, determines that it is needed in the Fire <br />Service Area. Copies of all mutual aid agreements shall be filed with City and Fire <br />Department shall keep City informed of the requirements, rules and regulations of all <br />mutual aid agreements to which it is a party. <br />8.0 Annual Submittals. The following items shall be submitted to City; <br />A. Operating Budget. On or before July 1s' of each year, Fire Department will submit <br />an annual operating budget for the period January 1 through December 31 of the <br />following year. The operating budget will be itemized for each station. City will make <br />Bi-Annual operating budget payments on or before January 151h and July 151h <br />B. Fire -Year Capital Improvement Budget. On or before July 15S of each year and <br />along with the operating budget, Fire Department shall submit a five-year capital <br />improvement budget which itemizes vehicles and equipment which are estimated to <br />cost more than $10,000.00. <br />C. Annual Audit. On or before July 15S of each year, Fire Department shall provide a <br />copy of the previous year's annual certified audit. <br />4 <br />
