DRAFT 1. Reaistration. Persons registering under this ordinance shall file a sworn statement with the City . Administrator's office on a form to be furnished by the City. The sworn statement shall proyide: A. The name, physical description and drivers license number of the person registering. B. The permanent home address of the applicant, the address of the place of business or the firm or occupation which the applicant represents, and in the case of transient merchants the local address from which proposed sales will be made; C. A brief description of the nature of the business and the goods to be sold; D. If employed, the name and address of the employer, together with the credentials therefrom establishing the exact relationship; E. The length of time for which the right to do business is desired; F. The place or location where the goods proposed to be sold are manufactured, stored and the manner in which goods shall be delivered to the customer; . G. A photograph of the applicant, taken within sixty (60) days immediately prior to the date of filing of the application, which picture shall be approximately 2" X 2" showing the head and shoulders of the applicant in a clear and distinguishing manner; H. A statement as to whether or not the applicant has been convicted of any crime, misdemeanor, or yiolation of any municipal ordinance, other than traffic violations, the nature of the offense and the punishment or penalty assessed therefor; I, A list including the name, address, phone number and age (if under 18 years) of all persons working for or assisting in the described actiYity. J. In cases of transient merchant sales, the applicant must also provide: i) Proof of appropriate permission to operate on proposed site. ii) A copy of the form or indiyiduals sales tax permit. The sales tax permit must be posted at the site; and . K. The last five (5) municipalities wherein the applicant has worked before coming to this City.
