CHARITABLE ~:a.M1>.LING <br /> Ordinance 4 - Division 5 <br /> Applicant must first submit the Minnesota Lawful Gambling <br /> application (Orange color LG214), and the Lease Agreement <br /> (LG202) . <br /> City staff will confirm: <br /> a. Lease agreement signed by the establishment where <br /> business will be conducted. (ie: Blue Fox, Big Ten <br /> Supper Club, Pot O'Gold Bingo Hall, etc) . <br /> b. Complete Investigative form(s) on all officers of the <br /> organization at the time the applicant(s) brings in the <br /> request. The Investigative form does ask for <br /> information that is not provided on the Gambling <br /> Control Board forms. <br /> c. Fax the Investigative form to the Sheriff's Office. <br /> d. Verification of non-profit status. <br /> e. Financial preliminary report. In addition, a report is <br /> due on or before February 1st of each year indicating <br /> how the net profits derived from lawful gambling <br /> activities conducted by the orginization have been <br /> . spent for lawful purposes. <br /> After confirmation: <br /> a. Prepare Resolution for agenda packet. Have the Mayor <br /> sign two originals: <br /> 1. One for applicant to submit to the Gambling Board. <br /> 2 . One to Resolution book. <br /> b. Administrator sign the (orange) LG214. <br /> c. Make copies of all for the Charitable Gambling file. <br /> NOTE: By ordinance the City of Arden Hills allows no more than <br /> nine (9) lawful gambling licenses of all classes. No more than <br /> seven (7) Class A licenses shall be issued and outstanding at the <br /> same time. <br /> Class A - Bingo License <br /> Class B - Pull tabs <br /> . <br />
