(6 ) The place or location where the goods proposed to be sold are manufactured, stored and the- manner in . which goods shall be delivered to the customer; (7) A photograph of the applicant, taken within sixty (60) days immediately prior to the date of filing of the application, which picture shall be approximately 2" X 2" showing the head and shoulders of the applicant in a clear and distinguishing manner; (8) A statement as to whether or not the applicant has been convicted of any crime, misdemeanor, or violation of any municipal ordinance, other than traffic violations, the nature of the offense and the punishment or penalty assessed therefor; (9 ) A list including the name, address, phone number and age (if under 18 years) of all persons working for or assisting in the described activity; (10 ) In cases of transient merchant sales, the applicant must also provide; . i) Proof of appropriate permission to operate on proposed site. ii) A copy of the form or individuals sales tax permit. The sales tax permit must be posted at the site; and (11) The last five (5) municipalities wherein the applicant has worked before coming to this City. Section 18-07. Exemotions: The provisions of this Section of this Chapter shall not apply to the following: (a) Bona fide residents of the state selling products of the farm and garden occupied and cultivated by themselves provided that such sales can be conducted without creating traffic hazards; (b) Children, age eighteen or younger, soliciting for school sponsored activities; (c) IlGarage" or HEstatell sales; . (d) The acts of merchants or their employees in delivering goods in the regular course of business;
