To complete the contract fonn, I will need the following information about the fiscal host: <br />I Correct, complete legal name <br />I Mailing address <br /> Minnesota tax ID number <br />I Federal employer ID number if the host has one <br /> Evidence that the fiscal host is in compliance with state workers' compensation <br /> requirements (a copy of a documentation form is enclosed) <br />I . If you are not sure who has authority to sign contraCts on behalf of the fiscal host, you will <br /> save some time by finding out now. A resolution will have to be adopted by the governing <br />I body that authorizes that person to sign any or this specific contract on behalf of the local <br /> government. A sample resolution will be sent to you with your draft contract. <br />I . The Board intends to issue you a lump-sum check as soon as the contract is executed. <br /> Under the terms of the contract, you will be providing us with quanerly reports telling us <br /> what progress you have made on your project and how you have spent the grant funds to <br />I dare. If for some reason you do not fully expend the grant, you will be required to refund <br /> any unspent money to the Board. <br />-: . As we informed you earlier, if you do not comply with the provisions of the contract, you <br /> will be required to repay the grant. For Service Budget Management Model and <br /> Cooperation Planning grants, the Board may require you to repay all or pan of the grant <br />I if you do not implement the model or plan. <br /> . I f you decide to subcontract any of the project activities to someone else not already <br />I described or named in your application, you must get written approval from us first. <br />I The next step in the process is that I will draft a grant contract based on the enclosed <br /> standard fonnat and your grant application, and forward the draft to you for your review <br /> and comment. You do not need to take any further action at this time. The Board will be <br />I issuing a statewide press release next week; you may wish to do your own local publicity, <br /> I look forward to working with you as you implement your project.
