<br /> . <br /> EXIllBIT B - <br /> This Exhibit B is an addendum to that certain lease agreement relating to 3776 Connelly Avenue, <br /> Arden Rills, MN and dated as of 3/28/95 , by and between Earthstar Project, Inc., <br /> lessee, and Goldie K Siedow, lessor. <br /> BINGO HALL RULES <br /> 1. Access to Buildinl!. Lessees will have access to the building from one hour before until one <br /> hour after scheduled bingo events. Access at other times may be obtained by permission of <br /> lessor. <br /> 2. Access to Office. The doOt to the office must remain closed and locked during bingo events. <br /> 3. EmDlovee Parking. Employees must park at the south end of the building. <br /> 4. Uniformity. Lessees must coordinate bingo programs, package prices, payment of shared <br /> supplies and services, and employee dress. Present dress code requires black pants (not jeans), <br /> black or white shoes, and plain white shirt. Vests are optional. Dress code must be enforced. <br /> 5 Smoking and Eating. Employees may not smoke or eat meals in the bingo area while on duty. <br /> 6. Cleaning. Lessees must clean tables after bingo events and pick up refuse from floor after - <br /> morning and afternoon sessions. <br /> 7. Insurance. Lessees must provide certificate ofliability insurance coverage in the amount of <br /> $500,000 minimum. Lessor shall insure building and contents against fire damage. <br /> 8. Buildinl! Security. Gambling manager or assistant gambling manager must personally <br /> supervise locking all doors and anning burglar alarm after last bingo event of the day. <br /> - <br /> 9. Responsible Parties. Each lessee must furnish lessor with up-to -date list of all organization - <br /> officers complete \vith home addresses and day and evening phone numbers. <br /> 10. Televisions. Radios and Telephones. Lessees and their employees may not have personal I <br /> televisions or radios on the premises during bingo events. Lessees and their employees may not <br /> use the house telephones for personal calls. I <br /> 11. Rel!Ulation ofReat and Air Conditioininl!. No one other than the owner shall, under any <br /> circumstances, have access to the thermostat controls. Anyone found tampering with the I <br /> thermostat controls shall be asked to leave the premises and will not be allowed access to the <br /> premises in the future. I <br /> 12. Damage to Property. The lessees are responsible for any and all damage done to the premises <br /> and/or property within the premises by their employees during the respective lessee's bingo .. <br /> I <br />
