City of Arden Hills <br />RFP for Recvclina Services <br />Form E: Price Worksheet <br />Instructions <br />All Respondents must fill out at least one set of these price worksheets for the "base <br />scenario" for fixed fees (without risk/revenue sharing). All parts of the price worksheet <br />must be completed. <br />To be responsive, the City requires proposed Recycling service prices for: <br />• RDUs, <br />• MDUs and opt -ins; <br />• Three (3) year vs. five (5) year contract terms; and <br />• Annual Event Cleanup. <br />Optional: Curbside Bulky Waste Collection Scenario and Risk/Revenue Sharing. <br />In addition, Respondents also may complete optional, alternate scenario price <br />worksheets. Simply copy these pages and clearly mark your additional alternate <br />scenario proposal as such. Respondents may submit multiple scenarios; there is no <br />limit. <br />Respondents may fill in the price worksheets below or use their own form. However, the <br />contents in the price worksheets below must be included if alternative forms are <br />submitted. When completed, print and sign the final forms for submittal with your <br />proposal. You may also print out this document, fill in your numeric answers by hand, <br />and then sign the price worksheets. All forms completed manually must be clear and <br />legible. <br />All price worksheets shall be executed by the authorized official to bind the Respondent <br />If alternate proposal(s) are submitted, a separate Form E — Price Worksheet must be <br />included, labeled "Alternate" with an appropriate title, that clearly specifies the proposed <br />costs of any such alternate provision. <br />In all cases, the City will own the carts for residential customers unless otherwise noted. <br />The Contractor is required to store and handle the distribution of extra carts as well as <br />field cart requests from residents. Alternate proposal scenarios may be proposed if the <br />Respondent chooses to offer a variation to the current Proposal system: The proposal <br />scenario includes a weekly single -stream collection and processing system, whereby <br />residents are instructed to commingle all groups of City -Designated Recyclable <br />Materials in a lidded, wheeled Recycling Cart. The City owns all of the Recycling Carts. <br />The Contractor will store, manage and distribute carts to residents. Basic design and <br />operating details of the current systems are assumed within this single -stream scenario, <br />with the exceptions of enhanced public education and enhanced Contractor reporting <br />requirements. <br />Additional alternates not specified above that add value (e.g., additional services) are <br />welcomed as long as a Proposal in accordance with the RFP and draft contract is <br />Form E - 1 <br />
