Any falsification of information on an application shall <br /> I <br /> be cause for denial, suspension or revocation of a license issued or to be issued hereunder. <br /> I <br /> I A. Business License. Every application for a business license required under this Chapter <br /> I shall be made to the Administrator on such forms as he or she may from time to time <br /> require. In addition to such information as the Administrator may require in such <br /> I application form, it shall also include at a minimum the following items of information or <br /> I documentation: <br /> I I. Whether the applicant is a natural person, a corporation, a partnership or other <br /> .. form of organization. <br /> I 2. If the applicant is a natural person: <br /> I <br /> I a. The true name, place and date of birth, street address and phone number of the <br /> applicant; <br /> I <br /> I b. Whether the applicant is a citizen of the United States; <br /> I c. Whether the applicant has ever used or has been known by a name other than <br /> I its true name, and if so, what was such name(s) and information concerning dates <br /> ,. and places where used; <br /> I 3 - 40 <br />
