UNPAID LEAVE OF ABSENCE <br /> I Upon consideration of a formal written request by an employee, leave of absence without <br /> pay may be granted at the discretion of the City Administrator based on consideration of <br /> the following factors: <br /> I 1. municipal service needs; <br /> 2. the employee's performance record and length of service; <br /> I 3. the reason(s) for the request; <br /> 4. any other relevant information. <br /> I If granted, unpaid leave of absence may not exceed a period of ninety (90) continuous <br /> calendar days. An extension beyond that period may be granted at the discretion of the <br /> City Administrator; however, an unpaid leave of absence shall not exceed one (1) <br /> I calendar year, except when the employee is in the military service or has been disabled <br /> while performing job duties in the municipal service. Benefits including vacation, holidays, <br /> sick leave or other forms of indirect compensation shall not accrue during a period of <br /> I unpaid leave of absence. Employees shall not be credited with service accrual while on <br /> unpaid leave of absence. During the first calendar month or portion thereof during which <br /> an employee began an unpaid leave of absence, the employee may continue to be <br /> Ie covered by group health insurance, but will be responsible for paying one hundred (100%) <br /> percent of the premium costs.
