11 <br />coupons to Qualified Beneficiaries and Continuation Participants. If <br />Employer and/or Plan Administrator’s notice to Benefit Extras is not timely, <br />Employer and/or Plan Administrator shall hold Benefit Extras harmless with <br />respect to any inaccurate information provided by Benefit Extras and shall <br />compensate Benefit Extras for any corrective action required (e.g., re- <br />distribution of notices or payment coupons to Qualified Beneficiaries and <br />Continuation Participants). Employer and/or Plan Administrator shall <br />indicate whether the Continuation Coverage Premium includes the 2% <br />administrative fee allowed with respect to certain types of Continuation <br />Coverage. Employer and/or Plan Administrator shall also notify Benefit <br />Extras of any special grace periods applicable to the payment of the <br />Continuation Coverage Premium under the Plan. Benefit Extras shall be <br />entitled to rely on such information. Benefit Extras shall be released and <br />relieved of all liability related to, and shall be indemnified by Employer and <br />Plan Administrator with respect to, Employer and/or Plan Administrator’s <br />failure to comply with the notice requirement contained herein. <br />F. Alternative Coverage. Employer and/or Plan Administrator must notify <br />Benefit Extras of any Alternative Coverage that impacts the services that <br />would otherwise be provided by Benefit Extras under this Agreement. If <br />such Alternative Coverage increases the amount of work Benefit Extras <br />must perform to comply with this Agreement, Benefit Extras reserves the <br />right to charge additional administrative service fees hereunder as agreed <br />by the parties. Benefit Extras shall be entitled to rely on the information <br />provided by Employer and/or Plan Administrator regarding Alternative <br />Coverage. <br />G. Continuation Coverage Determinations. Through this Agreement, Plan <br />Administrator delegates to Benefit Extras authority to make the described <br />determinations related to Continuation Coverage under the Plans. If Plan <br />Administrator disagrees with Benefit Extras on a particular determination, <br />Plan Administrator shall immediately notify Benefit Extras, in writing, of such <br />disagreement and direct Benefit Extras regarding the situation. Plan <br />Administrator shall be solely responsible for the final determination of such <br />a claim, which shall be communicated in writing to Benefit Extras. Benefit <br />Extras shall be entitled to rely on the final determination made by Plan <br />Administrator. <br />H. Termination of Coverage. Upon the occurrence of an event triggering a <br />loss of coverage under the Plan, Employer or Plan Administrator shall <br />complete and submit to insurance carriers and/or third-party service <br />providers any documentation required to terminate such Covered <br />Individual's coverage under the Plan.
