<br /> -~--- <br /> PROPOSED SCOPE OF SERVICES <br /> Old City Hall, Arden Hills, Minnesota . <br /> June 9, ]998 <br /> 1.0 ASBESTOS SURVEY <br /> Maxim will conduct an inspection of the building in accordance with Asbestos Hazard Emergency <br /> Response Act (AHERA) and Minnesota Department of Health (MDH) requirements. All sampling <br /> activities will be performed by a Minnesota certified Asbestos Inspector. All samples of bulk <br /> suspect asbestos-containing materials (ACM) will be collected in accordance with established <br /> chain-of-custody protocol and analyzed in one of our National Voluntary Lab Accreditation <br /> Program (NVLAP) accredited laboratories. <br /> We will require access to all areas in the building, including crawl spaces, closets, etc. We will <br /> investigate, under the direction of your representative, inaccessible areas, such as the inside of <br /> walls, to determine if ACM are present. However, we will only conduct destructive sampling at <br /> the request and direction of your representative. We will limit the area of destructive sampling <br /> at each location to one square-inch unless otherwise directed. Maxim will not be responsible for <br /> repairing the area of any sample location. However, Maxim will be responsible for the cleaning <br /> of accidentally released bulk material during sample collection. In addition, Maxim will <br /> temporarily patch the covering of thermal insulation with encapsulant and/or a good grade of duct <br /> tape. . <br /> Bulk sampling will be performed as follows: <br /> Friable Suifacing Materials: Three, five or seven samples from each homogeneous area, <br /> depending upon the size of the homogeneous area. <br /> Thermal Systems Insulation: At least three samples from each homogeneous area. <br /> Miscellaneous Materials: At least three samples from each homogeneous area of friable <br /> and non-friable miscellaneous material (floor tile and mastic, ceiling tile, roofmg materials, <br /> cement board, etc.). <br /> The homogeneous areas of suspect materials present will be estimated. The materials will be <br /> classified according to AHERA guidelines and an estimated minimum number of samples will be <br /> collected. Samples from each material group will be analyzed by polarized light microscopy <br /> (PLM) using a positive stop method. If a sample is tested positive, other samples from that <br /> homogeneous area will be held, but not analyzed. <br /> 2.0 PROJECT SCHEDULE <br /> Maxim is prepared to commence work on the Asbestos Survey within 5 working days after . <br /> receiving notification and written authorization to proceed.
