APPLICANTS WILL LEASE SPACE ON mE <br />TOWER AND WITHIN TIlE GROUND MOUNTED STRUCTURE, AS <br />SPECIFIED IN TIlE LEASE AGREEMENT. NO ADDITIONAL GROUND <br />STRUCTURES WILL BE PERMITTED ON THIS SITE. <br /> <br />7. SPECIAL REQUIREMENTS FOR CITY WATER TOWER SITES. THE PLACEMENT OF <br />LICENSED COMMERCIAL WIRELESS TELECOMMUNICATION SERVICES INCLUDING <br />CELLULAR, PERSONAL COMMUNICATION SERVICES (PCS), SPECIALIZED <br />MOBILIZED RADIO (SMR), ENHANCED SPECIALIZED MOBILIZED RADIO (ESMR), <br />PAGING AND SIMILAR SERVICES ON WATER TOWER SITES WILL BE ALLOWED <br />ONLY WHEN THE CITY IS FULLY SATISFIED TlIAT THE FOLLOWING <br />REQUIREMENTS ARE MET: <br />A. TIlE APPLICANT'S ACCESS TO mE FACILITY WILL NOT INCREASE THE RISKS <br />OF CONTAMINATION TO TIlE CITY'S WATER SUPPLY; <br />B. THERE IS SUFFICIENT ROOM ON THE STRUCTURE AND/OR ON TIlE GROUNDS <br />TO ACCOMMODATE THE APPLICANT'S FACILITY; <br />C. THE PRESENCE OF mE FACILITY WILL NOT INCREASE THE WATER TOWER OR <br />RESERVOIR MAINTENANCE COSTS TO TIlE CITY; AND <br />D. THE PRESENCE OF TIlE FACILITY WILL NOT BE HARMFUL TO TIlE HEALTH OF <br />WORKERS MAINT AlNlNG THE WATER TOWER OR RESERVOIR. <br /> <br />8. <br /> <br />Documentation Required. Installation and operation of an antenna, dish antenna or tower shall be <br />pennitted only after application has been made and approval has been granted as defmed in <br />Section VIII for a Special Use Pennit. The following documents shall be submitted with the <br />application: <br /> <br />a. Site Pian. The following information on the subject property shall be shown: <br /> <br />(1) A plot plan drawn to scale. <br /> <br />(2) Site and building dimensions. <br /> <br />(3) Location of buildings on the site and on adjoining sites within one hundred feet (100'). <br /> <br />(4) Location of site improvements, including drives, parking lots or structures, landscape <br />areas and any other site features that will aid the City in evaluating the application. <br /> <br />(5) Location and height of proposed antenna, dish antenna or tower. <br /> <br />(6) Elevation drawing of proposed antenna, dish antenna or tower. <br /> <br />b. Site Elevation Drawings and/or Site Section Drawings. Drawings shall be provided that show <br />how the screening will effectively be accomplished as defined in (6) above.
