Moore; Operations & Maintenance Director ~ j <br /> <br />State of Minnesota OSHA Inspection <br /> <br />,~ <br /> <br />TO: <br /> <br />FROM: <br /> <br />SUBJECT: <br /> <br />Backl!round <br />On Monday July 1, 2002 the State of Minnesota OSHA Department conducted an <br />inspection ofthe 0 & M facility. Ms. Cyndi Benjamin was the inspector. The City of <br />Arden Hills was issued with four Citations. Those four Citations carry penalties/fines <br />totaling $550.00. The Citations with their fine amount are listed below; <br />1. The upper mezzanine area of the shop, where the Yale two-ton hoist is located, <br />was not provided with a secured guard on the open side ofthe platform. Fine of <br />$150.00. <br />2. The Yale two-ton hoist located in the shop was not inspected for defects at daily <br />to monthly intervals. Fine of $150.00. <br />3. The electrical panel located in the shop area was blocked with particle board, <br />wood, etc. Fine of$ 100.00 <br />4. The Craftsman radial arm saw was not installed in such a manner that would <br />allow the cutting head to return to the starting position. Fine of$150.00. <br /> <br />Action Taken <br />Staff has corrected all of the four Citations listed above within two days. Here are the <br />actions taken; <br />I. A secure guard on wheels was made by Staff and installed in the upper <br />mezzanine. Staff orientation was held on proper operation. <br />2. A clipboard with the necessary inspection documentation was installed by the <br />Yale two-ton hoist. Staff orientation was held on proper inspection procedures. <br />3. The area around the electrical panel was cleared. Signage was posted to remind <br />Staff to keep this area clear. <br />4. The Craftsman radial arm saw was removed from the shop. No company makes <br />a radial ann saw with an automatic return for the cutting ann. <br /> <br />Recommendation <br />Staff recommends the payment of$550.00 to the State of Minnesota Department of <br />OSHA. <br />
