An ownership report, prepared by a licensed abstractor, indicating the ownership <br /> <br />of all parcels within five hundred (500) feet of the premises sought to be licensed, <br /> <br />accompanying the application. <br /> <br />13. Such other information as the Administrator or the Council may require from <br /> <br />time to time. <br /> <br />B. Personal Service Licenses. Applications for masseur or masseuse licenses provided for <br /> <br />in this Section shall be made to the Administrator on such fortns as he or she may from <br /> <br />time to time provide. In addition to such information as the Administrator may require, <br /> <br />the application shall include or be accompanied by: <br /> <br />1. All of the information required under Subpart A. 2. <br /> <br />2. Whether the applicant has ever had a similar license denied, revoked or suspended, <br /> <br />explaining the reasons for such denial, revocation or suspension. <br /> <br />3. What other communities the applicant has been licensed by during the preceding <br />five (5) years. <br /> <br />4. Written proof of age of at least eighteen (18) years. <br /> <br />3 - 48 <br />
