<br />2.2.2 Workers' Compensation and Emplovers Liability Insurance. Consultant shall <br />maintain workers' compensation and employers' liability insurance, (or employers "Stop Gap" <br />liability insurance for monopolistic states) with the following minimum limits: <br />Part One: Statutory <br />Part Two: $100,000 <br />$500,000 <br />$100,000 <br /> <br />Bodily Injury by accident <br />Bodily Injury by disease <br />Bodily Injury by disease <br /> <br />Each accident <br />Policy limit <br />Each employee <br /> <br />2.2.3 General Liabilitv Insurance. Consultant shall maintain general liability insurance <br />(written on an occurrence form) with coverage to include; Premises/Operations, Personal Injury, <br />Products and Completed Operations and Contractual Liability. City shall be named as an <br />additional insured under Consultant's general liability insurance, and such coverage shall be <br />primary and noncontributory to all other insurance or self insurance maintained by the City. <br />Limits ofliability shall not be less than: <br /> <br />$],000,000 each occurrence (Bodily Injury & Property Damage Combined Single <br />Limit, including coverage for liability assumed under contract and acts of <br />independent contractors) <br /> <br />$1,000,000 general aggregate (per Job Site) <br /> <br />$1,000,000 Personal Injury Liability, as usually provided under the standard ISO <br />Commercial General Liability Coverage Form CG 00 OJ <br /> <br />$1,000,000 Products and Completed Operations Aggregate <br /> <br />$5,000 Medical Payments <br /> <br />2.2.4 Automobile Liabilitv Insurance. Business Automobile Liability Insurance <br />covering all owned, non-owned and hired automobiles used in connection with the work, with <br />combined single limit coverage for bodily injury and property damage of not less than <br />$] ,000,000 per accident. <br /> <br />2.2.5 Evidence of Insurance. The above insurance shall be maintained in companies <br />]awfully authorized to do business in the state in which the Project is located and which are <br />reasonably acceptable to City. Prior to commencing the Services, Consultant shall furnish to <br />City and a certificate reflecting such insurance (ACORD form or equivalent) to be in force and <br />providing that it will not be canceled or materially changed without at least 30 days' prior written <br />notice to City.
