<br />GoodPointe Technology Copy-Page 1 of 1 <br />(please authorize and return to GoodPointe Technology) <br /> <br />Proposed Scope of Services - City of Arden Hills <br /> <br />GoodPointe Technology proposes to provide the City of Arden Hills with the following services: <br /> <br />Task 1.0 Pavement Condition Survey and Data Entry <br />The Pavement Condition Index (PC I) data collection survey will cover the selected streets within <br />the City of Arden Hills pavement network. The surface condition survey proposed in this task is <br />a walking survey in which 100 percent of the street surface will be evaluated. <br /> <br />At a minimum, the following bituminous pavement distress types will be collected as part of the <br />PCI Pavement Condition Survey: <br /> <br />Alligator Cracking <br />Block Cracking <br />Bumps <br />Corrugation <br />Depression <br />Edge Cracking <br />Flushing <br />Heaving <br />Lane-Shoulder Drop Off <br />Longitudinal Cracking <br /> <br />Patching <br />Polished Aggregate <br />Potholes <br />Railroad Crossing <br />Raveling <br />Reflective Cracking <br />Rutting <br />Shoving <br />Slippage Cracking <br />Transverse Cracking <br /> <br />GoodPointe Technology will enter all the data of the pavement condition survey, calculate the <br />PCI and then deliver the updated databases to the City for installation. The City will be able to <br />run system reports and budget scenario runs immediately after the installation of the updated <br />databases. <br /> <br />Basis of Payment: <br /> <br />Task 1,0 Pavement Condition Evaluation Survey and Data Entry <br />The pavement condition survey data will be conducted in the field on the selected pavement <br />sections within the City's jurisdiction and entered into the City's ICON program at a unit cost rate <br />of $225 per centerline mile, according to the terms specified in this proposal. <br /> <br />Total Estimated Costs: <br /> <br />Task Unit Units Total City <br /> Cost Renuested Cost SiQnature <br />1 $225 per centerline mile 31 $6,975.00 <br /> <br />After reviewing this document, please sign both copies.
