Grant Agreement Number � �� � C.�' <br /> Between ttie Minnesota Depariment of Health and The City of Centervil)e <br /> l�innesota Bepartment of Health <br /> Grant Agreement <br /> �HIS GRANT AGREEMENT, and amendments and supplements thereto, is between the State of Minnesota, <br /> acting through its Commissioner of Minnesota Department of Health (hereinafter "STATE") and the City o� <br /> Centerville, Minnesota, an independent organization, not an employee of the State of Minnesota, address, 1880 <br /> Main Street, Centerville MN 55038 (hereinafter "GRANTEE"), witnesseth that: <br /> WHEREAS, the STATE, pursuant to Minnesota Statute § 144.145, the Commissioner of Health is authorized to <br /> prevent tooth decay through fluoride control, testing, and reporting; and <br /> WHEREAS, the SATE, pursuant to Minnesota Rule 4700.0030 provides municipal fluoridation regulatory <br /> details regarding fluoride content (average 1.2 mg/L within a range of 0.9 to 1.5 mg/L), chemical feed, test <br /> equipment, monitoring, and recordkeepiiig. Currently, over 4 million Minnesotans are benefiting from <br /> cominunity fluoridated water; and <br /> WHEREAS, the STATE, pursuant to Minriesota Statute § 144.0742, the Commissioner of Health is authorized <br /> to enter into contractual agreements with any Public or Private entity for the provision of statutorily prescribed <br /> public health service by the department; and is empowered to grant funds under the Community Fluoridation <br /> Grant Program to increase the number of Minnesotans receiving the oral health benefits of optimally fluoridated <br /> drinking water; and i <br /> WHEREAS, GRANTEE represents that it is duly qualified and willing to perform the services set forth herein. , <br /> � <br /> NOW, THEREFORE, it is agreed: ' <br /> L GRANTEE'S DUTIES GRANTEE shall: � <br /> Upgrade the fluoride feed equipment as detailed in the Community Fluoridation Grant Application which is I <br /> incorporated by reference. j <br /> Comply with the requirements described in Minnesota Statute §144.145 and Rule 4700.0030 which is , <br /> incorporated by reference. T11ese requirements include but are not limited to: <br /> • Daily monitoring of water production volume, fluoride chemical consumption, and distribution system <br /> fluoride concentration (an MDH interim policy allows municipalities having populations less than � <br /> 3,300 to record measurements of fluoride feed consumption and distribution system sample analysis <br /> results at least once during each calendar week); <br /> • Submission of monthly reports of water production, fluoride chemical consumption, and distribution <br /> system fluoride concentration to the MDH; <br /> • Quarterly collection and submission of a distribution system sample to the MDH Public Health <br /> Laboratory (PHL); . . <br /> • Daily (or weekly if using the MDH interim policy) distribution system samples must be collected at <br /> locations representative of the entire water supply. Quarterly distribution samples must be collected at <br /> locations representative of any portion(s) of the water supply affected by any improved fluoridation <br /> process equipment during the contract period. <br /> Recipients of funds intended for fluoridation process equipment must submit and receive approval of <br /> plans and specifications from the MDH Drinking Water Protection Section prior to installation.
