MN0020300 0611712014 12:32 PM . ..... . ...... - Incident Narrafive 2 On Q6-17-2014 at 1125 hours, I was dispatched to Meadow lane and Centerville road, on a dog bi;te. Upon arrival, I met with the victim, JERRY CONRAD ALBRECHT (DOB09-24-1943) who said he was biking by 6994 Centerville road when 6 large German Shepherd came out from behind a fence and bit him on his right upper thigh. ALBRECHT denied medical attention. ALBRECHT showed me the wound which was two small puncture holes. ALBRECHT said he bikes past the house frequently, and never sees the dogs but he has heard them bark a lot in the past. I then met with the owners of the dogs, STAN HASIAK (DOB:04-08-1950) and STEPHANIE SINAIDA HASIAK (DOBA 2-05- 1952) both parties reside at 6994 Centerville road. STAN said he was outside in his backyard moving his two german shepherds, HEIDI and SZARYK, from the backyard into their kennels downstairs. STAN said he forgot to close the gate near Centerville road and HEIDI got out an ran toward Centerville road and ALBRECHT, STAN said SZARYK followed HEIDI who was chasing ALBRECHT at this time. STAN said he saw SZARYK bite ALBRECHT in his leg, and took control of both dogs after the bite. STEPHANIE showed me current rabies vaccination (until 09-091-2016) for SZARYK. STEPHANIE also showed me their current set-up for containing HEIDI and SZARYK in the backyard. I explained the quarantine period to both HASIAK's, and both agreed to keep SZARYK under quarantine for the next 1 0-days (06-27-2014). SZARYK quarantined at 6994 Centerville road until 06- 27-2014. I took pictures of ALBRECHT's wounds, informed ALBRECHT that the dogs both had current rabies vaccinations, and told ALBRECHT to contact the police department if he chooses to get any medical attention or if he has any questions. Quarantine form completed, and pictures uploaded to CLPD server. Pending follow-up. CSO Frederickson #303 -10-
