DRAFT <br /> CITY OF CENTERVILLE <br /> ANOKA COUNTY, MINNESOTA <br /> APPROVAL OF CONDITIONAL USE PERMIT <br /> 1. Permit. Subject to the terms and conditions set forth herein, the City of Centerville <br /> hereby grants approval of a conditional use permit for: <br /> Ultimate Martial Arts, 6845 - 201h Avenue South to operate a commercial <br /> recreational martial arts studio and subject to the conditions of this permit. <br /> 2. Property. The permit is for the following described property: <br /> 6845—2e Avenue South, Ste. #160 <br /> PIN#R23-31-22-44-0008 <br /> 3. Conditions. This permit is issued subject to the following conditions: <br /> a) All activities of this business shall take place indoors. <br /> b) All proper permits associated with a tenants desire to reconstruct the internal <br /> footprint of the suite will be completed within 30 days of approval of this CUP. <br /> c) No parking permitted within 20' fire lane, driveway,road right-of-way and all <br /> parking to be located on premises. Parking spaces utilized between the hours of <br /> 4:00 p.m. and 10:00 p.m. Monday- Sunday are limited to 18. All vehicles in <br /> excess of 18 will be parked in the rear parking lot. <br /> d) Landowner and the business operator shall jointly and individually be responsible <br /> for the continuing conformance with the permit and that if the proposed use is not <br /> initiated within six months of this date, the permit shall be void. <br /> e) Landowner and the business operator shall jointly and individually be responsible <br /> for notifying the City of this business' cease in operation and prior to any like <br /> business leasing or purchasing this space or any other space within the above <br /> stated building a submission for Conditional Use Permit will be secured prior to <br /> tenant location and operation. <br /> g) The City reserves the right to annually review this permit for compliance with the <br /> conditions and may impose additional conditions to mitigate unforeseen <br /> problems. <br /> 4. Criminal Penalty. Violation of the terms of this permit is a criminal misdemeanor. <br />
