payments for the purpose of affecting any structure or function of the body, for any hospital or nursing charges, optometric, ophthalmologic, or auditory care, dental care, psychiatric care, prescription drugs, insulin, prescription eyeglasses, hearing aid appliances, and similar prosthetic devices, and medical related transportation expense; provided, however, that medical care shall not include any cosmetic procedure that is not medically necessary. (1) "Participant" means an Eligible Employee of the Employer who has satisfied the participation conditions of Article III. A person who becomes a Participant shall remain a Participant for the limited purpose of Plan claims procedures and determining any and all benefits that may be due under the Plan, until all benefits due the Participant under the provisions of the Plan have been paid to the Participant or otherwise have been satisfied. (m) "Period of _Coverage", with respect to any Plan Year, means the Plan Year; provided that, (i) for any Eligible Employee who becomes a Participant after the start of a Plan Year, the Period of Coverage shall mean the period. commencing on the effective date of the Eligible Employee's participation and ending on the last day of the plan year, (ii) in the case of health care reimbursement coverage under paragraph 5.2.2, the Period of Coverage for a Participant shall end upon the last day of the month in which termination of employment occurs or the Participant ceasing to be an Eligible Employee, except the Period of Coverage will include any period of continuation coverage elected and paid for by the Employee consistent with this Plan or applicable law, and (iii) with respect to health care reimbursement coverage under paragraph 5.2.2, the Period of Coverage shall not include any portion of the Plan Year for which the Participant does not pay the applicable Health Care Reimbursement Charge. (n) "Plan" means the "City of Centerville Cafeteria Plan" as set forth herein and as amended or restated from time to time. (0) "Plan Sponsor" means the City of Centerville (p) "Plan Year" means the twelve-month period ending each December 31. 4
