<br />f' <br /> <br /> <br />1880 Main <br /> <br />Gl Centervi[[e; <br /> <br />55038 <br /> <br />(651) 429-3232:1) 'Fa?( (651) 429-8629 <br /> <br />STATE OF MINNESOTA <br />COUNTY OF ANOKA <br />CITY OF CENTERVILLE <br /> <br />RESOLUTION #03 - 003 <br /> <br />A RESOLUTION AUTHORIZING THE MAYOR TO SIGN THE CAFETERIA <br />PLAN DOCUMENTATION <br /> <br />WHEREAS, the City Council of the City of Centerville has adopted a Cafeteria Plan, <br />effective January 1,2003, for the benefit of eligible employees of the City of Centerville; <br />and' <br /> <br /> <br /> <br />WHEREAS, said plan complies with the provisions of Sections 79, 105, 125, and 129 of <br />the Internal Revenue Code of 1986, as amended; and <br /> <br />WHEREAS, The Stanton Group has been selected as contract administrator for the <br />Flexible Spending Account component of the Cafeteria Plan; and <br /> <br />THEREFORE, BE IT RESOLVED, that the Mayor of the City of Centerville is <br />authorized to sign the City's Cafeteria Plan Document as presented to the City Council on <br />January 8,2003. <br /> <br />Passed by the City Council of the City of Centerville this 8th day of January, 2003. <br /> <br />Mayor, Terry Sweeney <br /> <br />Attest: <br />
