Attachment A <br /> City of Centerville <br /> Ground Water Monitoring Permit Conditions– December 10, 2018 <br /> MNDNR Water Appropriation Permit 1991-6246 <br /> MONITORING – PRODUCTION WELLS: The PERMITTEE shall obtain monthly water level <br /> measurements in Installation #1 (unique well number 511091) and Installation #2 (unique well <br /> number 512748). The water level information may be obtained using a steel tape. <br /> Table 1. Production Wells <br /> Well Unique Number <br /> Installation #1 - CJDN 511091 <br /> Installation #2 - CJDN 512748 <br /> Hand Measurement Frequency: <br /> Groundwater level measurements must be collected from the production well by hand <br /> MONTHLY. <br /> Accuracy: <br /> Groundwater levels must be measured from the top of the casing to the nearest hundredth <br /> of a foot. Measurements must be reported as depth from the top of the casing and as an <br /> elevation in NAVD88 or equivalent standard datum. <br /> REPORTING: <br /> Water level readings must be submitted electronically to the MNDNR Groundwater Level <br /> Coordinator at gwlevelcoorAnr65�state.mmus on an Annual basis by February 15 of the <br /> following year or upon request by MNDNR Ecological &Water Resources Division. The <br /> permittee shall utilize the Excel Spreadsheet MNDNR template format for reporting purposes. <br /> This template is available from the MNDNR Groundwater Level Coordinator. <br /> MONITORING PLAN REASSESSMENT: <br /> This monitoring plan can be reviewed and amended at any time by MNDNR Ecological & Water <br /> Resources Division. After five years of additional data collection and analysis, or a significant <br /> change in operations, the MNDNR will evaluate the information and consider changes in the <br /> monitoring requirements if warranted. <br /> Authorized Signature: X 0 A?
