City of Centerville <br /> Proposal for Preliminary Geotechnical Services <br /> AET Proposal No. 01-20372 <br /> February 18, 2019 <br /> Page 3 of 4 <br /> FEES <br /> Our services will be invoiced as a Jump sum cost. Based on the scope of services presented in this <br /> proposal, including installing the temporary piezometer and the extra trip for removing it, our not- <br /> to-exceed lump sum cost will be $4,450.00. This amount includes a$200.00 budget for laboratory testing <br /> of soils and the necessary MN Department of Health filing fees(see below). <br /> If soil conditions dictate that additional drilling is needed beyond our estimated boring depths, due to poor <br /> conditions or to evaluate other areas,this will be invoiced at a unit rate of$27.00 per linear foot. We will <br /> contract you regarding additional work, and receive your authorization to proceed, prior to performing <br /> additional services. If additional mobilizations of the drill crew and equipment are needed because of the <br /> additional drilling,these will be invoiced at a unit cost of$325.00 per trip. <br /> PRIVATE UNDERGROUND UTILITIES <br /> Before we drill, we will contact Gopher State One Call to locate public underground utilities. Gopher <br /> State One Call does not currently charge for this service; however, they will not locate private <br /> underground utilities or structures. Examples of private utilities include, but are not limited to, propane <br /> and some gas lines, sewer laterals, sprinkler systems, site lighting, and electric and data lines between <br /> buildings. It is the responsibility of the client and/or the landowner to clear underground private <br /> utilities,if any,in the vicinity of the boring locations. <br /> MINNESOTA DEPARTMENT OF HEALTH REGULATIONS AND FEES <br /> Effective July 1, 2017,the Minnesota Department of Health(MDH)has changed the borehole sealing and <br /> notification requirements. For sites where borings are drilled to a depth of 15 feet or deeper, all licensed <br /> drilling companies are required by law to submit written notification to the MDH prior to drilling, along <br /> with a fee of $75. If one or more of the boring depths vary by 25 feet, or more, the MDH requires an <br /> additional written notification and fee of $75 per boring. Projects that span multiple properties will <br /> require multiple notifications. The NIDH also requires that these borings be grouted and that a Sealing <br /> Record be submitted to the MDH, with a copy to you, after the borings are completed. The above fee <br /> estimate for our geotechnical services includes the MDH fee for the proposed scope of drilling; however, <br /> because final boring depths can change, for example, due to possible unanticipated poor soil conditions, <br /> the final MDH fee(including an administrative charge of$65 per notification)will be added, if necessary, <br /> to our final invoice to you. <br /> The MDH Sealing Record requires the Property Owner name and mailing address (the Property Owner <br /> will also receive a copy of the Sealing Record).
