<br />May 16.2006 <br /> <br />Re: SM Liquor Liability Insurance Renewal <br />Dear Lions Club: <br /> <br />The Liquor Liability Insurance for your Club comes up for renewal on June30, 2006. <br /> <br />We have made a change in the program. Effective \\oith the June 30, 2006 policy period, <br />we will be changing Insurance Agents and Company. <br /> <br />The new Agency \\ill be The Hendrickson Agency Inc., 6400 FLying Cloud Drive, Suite <br />160, Eden Prairie MN 55344; phone number 952/903-2341 (Ronnie), fax 952/903-2395 <br />or www.hendricksonagency.com. <br /> <br />The Insurance Company is Colony Insurance Company. AM Best rating A VIII. <br />The limit has been increase from $300,000 each occurrence and $600,000 aggregate per <br />club to $1,000,000 each occurrence and $2,000.000 aggregate per chili. <br />The new program includes; members, Lioness and volunteers as additional insured. <br /> <br />The Annual premium, which will be fully earned at inception of the policy will be <br />5141.50. This is reduction from last year plus better coverage. <br />It also includes 5 additional Insured per club. such as City of-.-. or etc. without charge. <br />Above that there is an additional charge of $1 03 .00. <br /> <br />Liquor Liability is automatically provided for any event held at your premise and/or event <br />held off premise. <br /> <br />This prosmn was approved by the Council of Governors in Rochester. <br /> <br />DOT NOT SEND your renewal to Corporate 4 Agency, as they will no longer be an <br />approved program after your expiration date. <br /> <br />SEND your renewal and check to : Burdell R Wessels, PDG <br />13214 Kerry Lane <br />Eden Prairie MN 55346 <br />MAKE YOUR CHECKS PAYABLE TO: The Hendrickson Agency Inc. <br />I will forward them and they will send you a binder/certificate. <br /> <br />If you have any questions, please call me at 952/934-1762, fax 952/944-0607. <br /> <br /> <br />1t <br />
