(G) WorkersÓ compensation insurance. The policy limits for workersÓ compensation insurance shall be as <br />provided for by state law. <br />(H) Insurance Certificate Requirements. Whenever an insurance certificate is required by this Chapter the <br />applicant shall file with the City Clerk a certificate of insurance showing (1) that the limits are at least as <br />high as required, (2) that coverage is effective for at least the license term approved, and (3) that such <br />insurance will not be cancelled or terminated without third daysÓ written notice served upon the City <br />Clerk. Cancellation or termination of such coverage shall be grounds for license revocation. <br /> <br />(I) Operating Manager. All corporations, partnerships and associations must designate an Operating <br />Manager. The Operating Manager must be a person designated by the licensed premises who is in <br />charge of day-to-day liquor sales. <br /> <br />(J) Execution of application. <br /> <br />(1) If the application is by an individual, it shall be signed and sworn to by such person; if by a <br />corporation, by an officer thereof; if by a partnership, by one of the partners; if by an <br />incorporated association, by the operating officer or managing officer thereof. If the applicant is <br />a partnership, the application, license and insurance policy shall be made and issued in the name <br />of all partners. It shall be unlawful to make any false statement in an application. <br /> <br />(2) Falsification is a cause for revocation or denial of the license, and is a criminal offense <br />punishable as a misdemeanor. <br /> <br />(3) Each person or business named in or as part of this application shall execute a written <br />authorization allowing the city and/or the Chief of Police to verify all information disclosed as <br />part of the application. <br /> <br />Ord. #13, adopted 08/28/2002, Ord. 2d # 13, amended 10/11/2006 <br /> <br />§ 111.11 DESCRIPTION OF PREMISES. <br /> <br /> The application shall specifically describe the compact and contiguous premises within which liquor <br />may be dispensed and consumed. Except as for otherwise provided for herein. <br /> <br />Ord. #13, adopted 08/28/2002 <br /> <br />§ 111.12 APPLICATIONS FOR RENEWAL. <br /> <br />(A) At least 90 days before a license issued under this chapter is to be renewed, an application for renewal <br />shall be filed with the city.
