DocuSign Envelope ID: 2BA5E40A-FF10-4F0E-ACB6-711F2F7F2DFF <br />Anoka County Contract #EC0009950 <br />STATEWIDE HEALTH IMPROVEMENT PROGRAM <br />MEMORANDUM OF UNDERSTANDING <br />This Memorandum of Understanding is entered into by Anoka County, a political subdivision of the State <br />of Minnesota, 2100 Third Avenue, Anoka, Minnesota 55303 ("County"), and City of Centerville, 1880 <br />Main Street, Centerville, MN 55038 (Contractor). <br />RECITALS: <br />(1) As Grantee, County has accepted grant funds from, and entered into a Grant Agreement with, the <br />Minnesota Department of Health based on Grantee's Work Plan. <br />(2) County included grant activities associated with implementing policies and practices that result in <br />development of MN Moves strategies. <br />(3) Contractor represents that it is qualified and willing to furnish these services. <br />(4) County wishes to establish a Memorandum of Understanding (MOU) with Contractor for these <br />services. <br />It is agreed and understood as follows: <br />1. TERM <br />1.1 The term for this Memorandum of Understanding begins on 4/1/2023 and ends on 10/31/2023, <br />regardless of the date of signatures. <br />2. JOINT EFFORT <br />2.1 County and Contractor will work together to support MN Moves in the community setting. <br />2.2 Activities may be guided by input from the Community Leadership Team. <br />2.3 County and Contractor staff agree to meet jointly as needed to resolve issues that may arise. <br />2.4 Activities will be developed based on SHIP guidelines. <br />3. COUNTY RESPONSIBILITIES <br />3.1 County agrees to: <br />3.1.1 Provide technical assistance and support. <br />3.1.2 Reimburse Contractor for purchase of hydration station drinking fountains in an amount <br />not to exceed $6,000.00. <br />3.1.3 Reimburse Contractor for purchase of bike repair stations in an amount not to exceed <br />$2,500.00. <br />
