DocuSign Envelope ID: 2BA5E40A-FF10-4FOE-ACB6-711F2F7F2DFF <br />Anoka County Contract #C0009950 <br />Attachment A <br />Scope of Services <br />The goal of the Statewide Health Improvement Partnership (SHIP) MN Moves context area is to increase <br />opportunities for physical activity, especially walking and biking, for all Minnesotans. MN Moves <br />integrates physical activity into daily routines such as walking or bicycling for recreation, occupation, or <br />transportation. MN Moves policies and practices in community design, land use, site planning, and <br />facility access have proven effective to increase levels of physical activity. <br />To support MN moves in the community setting, Anoka County SHIP will reimburse the City of <br />Centerville up to $6,000 for hydration station drinking fountains and $2,500 for bike repair stations to <br />increase accessibility in the parks. <br />• Anoka County SHIP will reimburse the City of Centerville for the purchase of hydration station <br />drinking fountains (up to $6,000). <br />• Anoka County SHIP will reimburse the City of Centerville for the purchase of bike repair stands <br />(up to $2,500). <br />• The City of Centerville will be responsible for installation of the benches and any related <br />expenses. <br />• The City of Centerville will be responsible for any costs related to maintaining the benches, <br />including any expenses related to repairs or replacement. <br />• The City of Centerville will foster ongoing communication with Anoka County SHIP and <br />participate in evaluation activities/provide data as needed. <br />
