SPECIAL ASSESSMENT AGREEMENT <br />WAIVER OF HEARING AND APPEAL <br />AGREEMENT made this ______ day of _______________, 2024, by and between the 2024 <br />City of Centerville, a Minnesota municipal corporation (ÐCityÑ), and Kylie Haffke and <br />Alexander Haffke (ÐProperty OwnersÑ), a married couple. <br />RECITALS <br />A.Property Owner(s)arethe fee owners of the following described real property, <br />located in the City of Centerville, Anoka County, Minnesota (ÐSubject PropertyÑ): <br />PIN #23-31-22-42-0015 LOT9, BLOCK 3, CENTER OAKS 1, <br />6908 Oak Circle EX RD SUBJ TO EASE OF REC <br /> <br />B. Property Owners request to install 60Ó of draintile/piping to direct stormwater into <br />the CityÓs concrete catch basinlocated in their rear yard to assist stormwater flowage away from <br />the home as per Exhibit A. <br />C.Property Owners have agreed to participate in the CityÓs program for cost share <br />(25% - City of CentervilleÓs Portion, ($2,365.62) and 75% - Property Owners Portion, <br />($7,096.88), authorized by City Resolution #13-022 & #23-017. Two bids have been submitted & #23-017 <br />by the property owners and the property owners have chosen the lowest bidder of a fee of <br />$9,462.50 (Exhibit B). <br />D.Prior to payment to the property ownerÓs chosen contractor, Schifsky Companies, <br />the City will receive a Lien Waiver anda FormIC324 from Schifsky Companies upon <br />completion of the projectand request for payment. Said payment will not exceed the amount <br />assessed through this agreement. <br />E.
