CITY OF CENTERVILLE <br />REQUEST FOR <br />COUNCIL ACTION <br />Agenda Item # Department: Requested Council Meeting Date: <br />January 22, 2025 <br />Administration <br />VII. <br />TITLE OF ISSUE: <br />Massage Therapist License - Soothing Sage Massage, LLC Renewal <br />BACKGROUND AND SUPPLEMENTAL INFORMATION: <br />The applicant, Ms. Jodi Cruz, AKA Ms. Jodi Day, has completed all applicable applications, paid associated <br />fees and a successful background check was completed by the Centennial Lakes Police Department. <br />It should be noted that Ms. Cruz, AKA Ms. Jodi Day, does have a criminal background. Charges are from <br />numerous year prior to licensing in Centerville. Previous consideration by Council and the City Attorney found <br />Ms. Cruz worthy of licensing.
