NOTICE OF CANCELLATION <br />Enter type of goods or services purchased: X____________________________________ <br />Date of Transaction: X_______________ <br />If you do not want the goods or services described above, you may cancel your purchase by mailing or delivering a signed and <br />dated copy of this cancellation notice or any other written notice, or send a telegram to Contractor no later than midnight within <br />three business days from the above date.
