The applicant was awarded on <br />th <br />05/11/2012. The applicant also provided a letter from M Health dated December 16, 2025, <br />indicating that she has completed a physical examination and is found to be free of any <br />communicable disease. <br />Criminal History <br />There has been no change since last renewal. Comprehensive checks of multiple law <br />enforcement databases show no concerns or contacts since last renewal. <br /> <br />Summary <br />The background investigation is now complete.
