<br />. <br /> <br />APPENDIX 8 <br /> <br />INSURANCE <br /> <br />The EMPLOYER will pay fifty percent (50%) of the individual health insurance <br />premium for permanent part-time employees up to a maximum of one hundred <br />dollars ($100.00) per month. <br /> <br />The EMPLOYER will pay a flat fee of one hundred fifty dollars ($150.00) per <br />month for permanent full-time employees electing single health insurance <br />coverage. Any amount over the $150.00 premium will be paid by the Employee. <br />Any excess amount not used for health insurance under the $150.00 flat fee can <br />be used to purchase additional life insurance or taken as deferred compensation. <br /> <br />The EMPLOYER will pay a flat fee of three hundred dollars $300.00 per month <br />for permanent full-time employees electing family insurance coverage. Any <br />amount over the $300.00 premium will be paid by the Employee. Any excess <br />amount not used for health insurance under the $300.00 flat fee can be used to <br />purchase additional life insurance or taken as deferred compensation. <br /> <br />-. The EMPLOYER shall provide for each employee the same Dental Insurance <br />coverage as current non-bargaining unit employees receive in accordance with <br />the City's current policy. <br /> <br />20 <br />
