PaJ( (651) 429-8629 <br /> <br />A RESOLUTION TO SELF-ADMINISTRATE AN EMPLOYEE BENEFITS PLAN <br />WITH ASSISTANCE FROM AFLAC <br /> <br />BE IT RESOLVED that the City of Centerville with assistance trom AFLAC will self- <br />administrate an employee benefits plan. <br /> <br />BE IT FURTHER RESOLVED that the following guidelines are established beginning <br />January 1, 2007 for employees of the City ofCenterville participating in the plan: <br /> <br />1. The city contribution for calendar year 2007 will be $880 per employee. <br />The contribution amount will be established on enmlaJ basis by the council. <br /> <br />2. The core plan that participants will be required to enroll in consists of the <br />following: <br />a. Single health coverage; <br />b. Employee dental coverage; <br />c. Employee life insurance $10,000. <br /> <br />3. Optional benefits offered to employees shall consist of the following: <br />a. Supplemental and dependent life coverage; <br />b. Dependent dental coverage; <br />c. Short-term disability; <br />d. Long-term disability; <br />e. Long-term care coverage. <br />f. AFLAC insurance coverage <br /> <br />4. Employees will be allowed to contribute pre-tax dollars to the following or <br />take the balance as taxable pay: <br />a. Health insurance premiums spending account; <br />b. Un-reimbursed health care spending account; <br />c. Day care spending account; <br />d. Deferred Compensation account. <br /> <br />S. The dollar limit for employee contribution to the un-reimbursed health <br />care spending account shall be set at: <br />a. $3,000 <br />
