<br />[Feb. 12, 2001] <br /> <br />APPENDIX B <br /> <br />INSURANCE <br /> <br />Effective January 1, 2001: <br /> <br />The EMPLOYER will continue to pay fifty percent (50%) of the individual health insurance-- <br />premium for permanent part-time employees up to a maximum of two hundred and seven <br />dollars ($207.00) per month. <br /> <br />The EMPLOYER will pay a flat fee of two hundred fifty-seven ($257.00) dollars per month for <br />permanentfull-time employees electing single health insurance coverage. Any amount over the <br />$257.00 premium will be paid by the EMPLOYEE. Any excess amount not used for health <br />insurance under the $257.00 flat fee can be used to purchase additional life insurance or taken <br />as deferred compensation. <br /> <br />The EMPLOYER will pay a flat fee of four hundred and seven dollars ($407.00) per month for <br />permanent full-time employees electing family insurance coverage. Any amount over the <br />$407.00 premium will be paid by the Employee. Any excess amount not used for health <br />insurance under the $407.00 flat fee can be used to purchase additional life insurance or taken <br />as deferred compensation. <br /> <br />The EMPLOYER shall continue to provide for each employee the same Dental Insurance <br />coverage as current non-bargaining unit employees receive in accordance with the City's <br />current policy. <br /> <br />The Employer/ Employee split of insurance premium increases will be eighty percent (80%) <br />Employer and twenty percent (20%) Employee of the lowest cost plan offered by the City. <br /> <br />All increases will be retroactive to January 1, 2001. <br /> <br />DEFERRED COMPENSATION PROGRAM <br /> <br />Employees choosing not to be covered by City paid medical insurance will be eligible to <br />contribute the full amount of the single premium towards a deferred compensation plan, <br />effective January 1,2001 through December 31, 2003. <br /> <br />19 <br />
